If you are travelling abroad and need vaccinations, ideally we need 2 months notice to process your vaccination request.
Please come in and collect a Travel Vaccination Form from the front desk, and complete as required. Or download the Travel Vaccination Form (DOC, 75KB) and return the completed form to the surgery. You can also use eConsult: latchfordmedicalcentre.webgp.com/show-travel-advice
Then we will arrange a telephone consultation with our Practice Nurse who will advise on which vaccinations are needed and they will arrange an appointment to have them done.
If the form is handed back in to us and it is too late for us to vaccinate you here, our Practice Nurse will give you information on private Travel Clinics.
