Sick notes are only issued following an illness that has already kept a patient off work for 7 days. More information about when you need a fit note.
If you need to provide a self-certification form to your employer for sickness you can either:
- Enter the details online and print it off
- Download the form and complete it by hand (we also have these forms available in our reception). Please download the form to complete by hand.
